Anyone graduating from Lakota High School 1997 or before print this transcript request (in pdf format) and mail to the address below or contact Judy Smith, 513-874-8390 (email judith.smith@lakotaonline.com) Due to this summer's building renovations, we will need to limit the days that transcripts are available in June and July.  Transcripts and/or graduation verifications will only be available on June 6, June 23, July 11 and July 23. This only applies to Lakota Grads who attended Lakota High School and graduated in 1997 or before. The Cost is $5.00 per transcript.
